In an effort to continue to streamline our processes, we have made updates to the Direct Deposit Form. The new Direct Deposit Form allows agents to submit updated banking information, along with the required supporting documentation, electronically.

As a reminder, this form is only to be used to update GoHealth’s records for carriers’ commissions paid by GoHealth, NOT carriers who pay commissions directly.
