Anthem Medicare | Temporary Appointment Process

Anthem is aware of current issues that our agencies are experiencing with NoMoreForms. In the meantime, they have provided a temporary solution for appointing agents while they work out the kinks. For the time being, please have agents complete this Anthem Appointment Request Form.

Once an agent completes the Wufoo form, they will be redirected to RightSignature where they will need to complete the background questions. If an agent would like to become appointed in FL, there will be a separate RightSignature link emailed to them that must completed in addition to the other forms in order to become appointed. Once an agent successfully submits all documents, our team will be notified and we will process the agent.

It is important to note that we will only be able to send this file on Thursdays. Any agent wanting an Anthem appointment will need to fully complete the request form prior to 1pm CST on Thursdays. If submitted after 1pm it will be included on the following weeks file.
